HospitalityCare doesn’t restrict your choice of health care providers. Your plan pays the same benefit no matter what doctor you choose. However, you’ll want to show your health care provider your HospitalityCare ID card at the time of service. Some doctors participate in the Beech Street Network and offer discounted fees for service, and will file claims for you.
If your doctor is not in the network, simply save your doctor's bill and receipts and file a claim form to HospitalityCare. The plan will then process the claim and mail you a check based on your benefit amount.
For prescriptions, be sure to show your ID card at the pharmacy. Participating pharmacies in the Caremark Pharmacy Network will provide you a discount at the time of purchase. Depending on the particular plan offered by your employer, you will either receive your benefit at the pharmacy or you will need to save your receipt and file your claim to HospitalityCare.
Your medical plan also includes discounts on vision care such as eye exams, frames, lenses and contacts, even if you did not elect vision coverage. Be sure to show your HospitalityCare ID card when you visit a participating Cole Managed Vision Network provider at more than 6,500 participating locations nationally (Pearle Vision, Lens Crafters, Sears, JC Penney, Target Optical and many others).
In addition to the discounts on vision services that are part of your medical coverage, if you elected vision coverage, you can choose one of these three benefit options:
Choice A: Eye Glasses
One pair of frames and two lenses (single vision, bifocal, trifocal or lenticular) per 12 months paid at 75 percent co-insurance after a $15 per purchase deductible, with a maximum benefit of $25 for frames and maximum benefits ranging from $35 to $75 for lenses.
Choice B: Non-Disposable Contact Lenses
Two lenses per 12 months paid at 75 percent co-insurance after a $15 per purchase deductible, with a maximum benefit of $95.
Choice C: Disposable Contact Lenses
Up to a 12-month supply per 12 months paid at 75 percent co-insurance after a $15 per purchase deductible, with a maximum benefit of $75.
