The Limited Benefit Program for Employees of Dunkin' Donuts Franchisees includes two different types of plans.
If your employer has 50 or under employees, your plan allows you to see any physician. Simply save your doctor's bill and receipts and mail in a claim form to PAI for the covered amount of the claim. When you visit a pharmacy be sure to show your ID card to fill a prescription and receive the Caremark discount. Save your receipts and fill out a claim form and send them in to PAI. For discounts on vision care and eye glasses and contacts, show your ID card to a Cole Managed Vision Network Provider.
If your employer has over 50 employees, your plan includes access to the nation’s largest independently owned preferred provider organization (PPO). Most physicians in the network will ask you only for the portion of the claim that is your responsibility, such as the $15 doctor's office visit copayment. PHCS Network
Private HealthCare Systems (PHCS) is the largest proprietary PPO in the country, with nearly 371,000 providers and more than 3,500 facilities. PHCS is very recognizable - nearly 6.5 million enrollees nationwide carry the PHCS identification card.

Pharmacy
With the Caremark Pharmacy Network, you have access to more than 58,000 pharmacy locations nationwide, where you can save up to 50 percent on generic drugs and up to 15 percent on brand name drugs.

Vision
Through the Vision One Eyecare Program under the Cole Managed Vision Network, your plan gets you discounts on vision care and prescription eyewear. The national network of providers includes well-known names such as Pearle Vision, Sears Optical, Target Optical, JC Penney and thousands of independent optometrists. Your plan also includes limited insurance benefits for eye exams, contact lenses and glasses.
